Are you eligible?

Before you begin the process of applying to St. Jude’s Community Homes, you should first determine whether you are eligible. You may qualify if you:
  • Live with a serious mental illness, mainly schizophrenia

  • Have a mental health professional that will refer you to St. Jude and continue working with you after you move in

  • Would like to live in a community and have the opportunity to interact with others

  • Have demonstrated the ability to manage your own medications

  • Have demonstrated the ability to live somewhat independently with the support of your mental health worker

  • Are not currently involved in the criminal justice system

  • Do not have a drug or alcohol dependency

  • Are a Canadian citizen or Landed Immigrant

  • Have a good reference from your previous two landlords and do not owe rent to any other housing authority

  • Do not own a house, or if you do, have agreed to sell it within six months of moving into St. Jude Community Homes

How to Apply

Choosing where you live is an important decision. As such, we will do our best to provide you with as much information as possible so that you can make the choice that is right for you. To apply to live in one of our apartments, there are a few steps you must take, which are detailed below.
  1. You need to be referred by a mental health professional
    All referrals are made through the Access Point central intake office during regular business hours. The phone number is 416-640-1934.

  2. Complete the Common Application Form
    The application form can be obtained through the Access Point office or can be downloaded from their website at www.theaccesspoint.ca.

  3. Come for a Tour
    Once we receive your application from the Access Point office, we will review it and confirm the information provided with the referrer. If we believe you meet our criteria, we will invite you to come for a tour.

  4. Vacancy
    When we notify the Access Point office that there is a vacancy in one of our apartment units, the Access Point office matches our vacancy with individuals on their waitlist who meet the criteria for our program and services and send us the application form.

  5. Participate in an Interview
    Following the tour, if you are interested in our programs and services, a date will be set for an interview. The interview consists of a meeting with members of the management team, you will be invited to stay for an evening meal and meet with some of our current residents.

  6. Successful Completion of our Intake Process
    Once you have successfully completed our intake process and have been offered an apartment a move in date will be chosen.
